Medications at School
All medicine (over the counter or prescription)
administered at school, must have a medication
authorization form on file in the clinic.
Prescription medicine must have a physician's
signature on the form before medicine can be
administered. All medicine must be in its original
container, with the child’s name on it. The form and
medicine must be brought to school by a parent or
other adult, and will be kept in the school clinic.
Under certain circumstances, students may keep
inhalers and epi-pens in their possession if it is
deemed necessary by his or her doctor. The
medication authorization form for this circumstance
must be signed by a physician as well. A copy of the
form will be kept with the student, while the
original will be kept in the clinic.
Students with allergies and/or asthma should have an
asthma and/or allergy plan on file as well.
STUDENTS CANNOT CARRY MEDICATION WITHOUT A
MEDICATION
